Our staff looks forward to meeting you and discussing your goals. We strive to keep our appointments on time as scheduled. Prior to your initial appointment, you will need to fill out patient forms. We recommend that you arrive at your appointment 15 – 30 minutes early so you have ample time to fill these out.

To secure your appointment, we require a valid debit or credit card on file. This information is kept confidential and is used solely to hold your appointment.
Consultations are provided at no charge. However, we do ask that you provide us with adequate notice if you need to reschedule or cancel.
If you need to cancel or reschedule your appointment, we request that you provide at least 48 hours' notice. This allows us to offer your time slot to another patient who may be on our waiting list.
Failure to provide 48 hours' notice will result in a $150 charge. This fee will be applied to your card on file.
If you do not show up for your scheduled appointment, the $150 no-show fee will also apply.
If you cancel less than 48 hours before your scheduled appointment, you will be charged the $150 cancellation fee.
We understand that emergencies and unforeseen circumstances arise. If you need to cancel or reschedule due to an emergency or other extenuating circumstances, please contact us as soon as possible. We will review these cases on an individual basis.
If a patient repeatedly cancels or fails to show up for scheduled appointments, we may require prepayment for future appointments or reserve the right to deny future bookings.
